一、学生要如实汇报最近在校学习状况

二、学生要接受本中心的入学检测

四、甲方责任和义务

1、  双方签订本协议后,甲方即为乙方建立文本档案。档案含学生情况登记表、委托辅

导协议及在本协议签订之日起至本协议解除前辅导过程中所产生的辅导方案、授课教师教案、授课计划、学生测试卷、学生不同时期存在的问题研究及分析报告等内容。

2、  甲方委派教育咨询师与乙方及乙方监护人沟通,全方面了解乙方学习情况、学校表

现、教育历史、过往辅导经历及亲子关系等真实情况,并作详细记录,存入乙方档案。

3、  根据对乙方的测试和咨询,甲方制定一个切实可行的辅导方案并告知乙方监护人。

4、  乙方向甲方按照约定交清费用,甲方根据乙方的实际情况每周为乙方提供课外辅

导,具体的授课方式和授课内容以及时间安排,由甲方根据乙方实际的情况而定。

五、乙方(乙方监护人)责任和义务

1、乙方及乙方监护人应到甲方规定地点报名,进行咨询和测试时,乙方和其监护人必

须到场。

2、咨询时,乙方及其监护人需提供甲方所需要的一切真实建档信息,以供甲方确定

乙方的辅导方案和授课计划。

3、甲方授课教师每次一对一授课完成后,甲方需要在甲方制定的课时确认单上签字认

可,作为甲方统计课时和确认乙方已上课时的依据。

4、乙方接受甲方辅导时,需接受授课教师的严格管理,乙方须接受甲方安排的学习内

容和进度,如需变幻须及时通知甲方,甲方认同后方可进行。

5、乙方监护人须及时反映乙方在学习过程中出现的新问题,以利于甲方进行个性化辅

导,,乙方需每月写一份个人月总结,总结上个月的学习情况和制定下个月的学习计划,

提供给甲方。

    6、乙方因特殊原因需要推迟授课时间计划的,需提前48小时以书面或者电话方式通知甲方并得到认可。

    7、乙方不得私下与甲方授课教师达成任何异于本协议规定的辅导计划,不得向甲方授

课教师垂询任何薪资数额情况,不得向任何第三方透露本协议内容。

六、违约责任

1、  学生档案作为本协议的一个重要组成部分,由甲方保管,直到双方协议终止。乙方

需提供给真实情况,双方均不得随意查询和更改档案内容,如果乙方需要可以进行部分复印。

2、  在协议执行期间,乙方如对甲方服务不满或有其他原因等情况下,可申请解除协议,

甲方将扣除实际已消耗总课时的一对一教师授课费(按本中心实际消耗课时数的单价等级计算或按辅导总费用与辅导总时间的比例),将剩余部分在7个工作日内全部退还乙方。

3、  若由于乙方提供不真实资料签订本协议的或由于乙方私下与授课教师达成任何异

于本协议规定的辅导计划、或若乙方向甲方垂询任何薪资数额行为,导致影响教学计划和效果的;以上行为导致后果由乙方自负,甲方可无条件解除协议,甲方不承担任何违约责任,并有权要求乙方赔偿损失。

4、 如果乙方小考分数没有达到双方所定的目标学校的分数线,甲方向乙方退回全额的

承诺保证金(不含基本课时费部分)。

5、  如因自然灾害等不可抗拒因素导致本协议无法继续履行,双方均不承担责任。

七、附加部分

1、本协议一式两份,双方各执一份,自签订之日起生效。辅导日期结束后,双方责任

和义务即自动终止。 

2、本协议履行期间如发生异议,由双方友好协商解决。

B When my son, Mark, was in the third grade, he saved all his pocket money for over three months to buy holiday presents. The third Saturday in December Mark said he had made his list and had twenty dollars in his pocket.
