一、对外联系材料及其提交要求

1、奖学金申请表(APPLICATION FORM FOR FOREIGN STUDENT SCHOLARSHIP):朝文填写;注意写明自己的朝语水平。

2、体检表(REPORT FOR MEDICAL EXAMINATION FOR FOREIGN STUDENT):中文填写;市级以上医院

3、进修生简况表:用中文详细填写;学历一栏请从“大学”添起，导师一栏可不填，需补习朝语的请注明补习时间;进修计划及要求需认真填写。

所有栏目毒必须用朝文或中文认真填写;务必工整、清晰

4、大学毕业证书、学位证书:复印件(加盖原样校章);仅限已参加工作留学人员。

5、单位推荐公函:朝文;仅限已参加工作的留学人员。

6、护照号码复印件:仅限“艺术专业学者”

7、国家公派留学人员信息一览表(中文):为方便联系，请详细填写，一式两份。

上述材料一律使用A4纸复印。请按照1-5的顺序装订整齐(若有朝方院校邀请信，请附复印件在最后)，一式三份(原件一份，复印件2份，均附贴相片)。
